Send: 1) This original rebate form (uh-oh - hope yours came with it on the top) If not, call the company and ask them about it.

2) a 3x5 card with your name, street address, city, state, zip code, phone number, email address and store where you purchased the product written on it.

 3) Dated store identified cash register receipt with UPC # written on the back from the Per Plus product you purchased from a participating retailer.
